A man in his 20s, 4 months post-op revision rhinoplasty. The patient had a previous rhinoplasty which left him with a foreshortened and overly rotated nasal tip with alar retraction (ability to see up the nostrils). He desired subtle lengthening and derotation of his nasal tip with correction of alar retraction to create a more masculine appearance of his nose. He underwent nasal tip derotation and lengthening with extended spreader grafts and alar retraction correction with alar contour grafts using MTF cartilage. The overall goal is a natural looking result which maintains a masculine appearance of his face.
